Americade Home Health Agency will provide home health care to eligible individuals based on the following criteria. Those who may receive our services include individuals who are currently under a physician’s care and who:
1. Meets requirement of home bound
2. Require intermittent nursing care
3. Need Speech, Physical, Occupational Therapy
4. Need Social Work Services

SUPPLIES AND EQUIPMENT
Americade Home Health Agency will arrange for all necessary supplies for the patient’s home and coordinate necessary equipment rentals through local companies.
